The Bureau of Labor Statistics report 13 workers lose their lives everyday in the United States. And nearly 3 million nonfatal injuries and illnesses occur annually. These events cause much suffering and great financial loss to workers and their families, and result in significant costs to employers and to society as a whole. Many more “near hits” or “close calls” also happen; these incidents could have caused serious injury or illness but did not, often because of sheer luck.
All incidents – regardless of size or impact – need to be investigated. The process looks beyond what happened to discover why it happened. This allows you to identify and correct shortcomings in your safety and health management programs and hopefully, prevent them from happening again.
